Frozen Embryo Transfer (FET) Timeline: What to Expect Step by Step

At Pacific Reproductive Center, we understand that the journey to parenthood is one paved with hope, resilience and often, many questions. Whether you are visiting us at our locations in Torrance, Glendale, Irvine or Corona, our mission is to provide more than just medical excellence; we provide a compassionate partnership.
One of the most common paths our patients take is the frozen embryo transfer (FET). Understanding the timeline of this procedure can help ease the anxiety of the unknown and empower you as you move toward your goal of a healthy pregnancy.
What is a frozen embryo transfer (FET)?
The frozen embryo transfer approach is a sophisticated fertility procedure that offers flexibility and high success rates. Research published in the JBRA Assisted Reproduction journal found that women who underwent in vitro fertilization (IVF) via frozen embryo transfer showed more successful fertility and pregnancy outcomes than those using fresh transfer, with significantly higher rates of clinical pregnancy, ongoing pregnancy and live births.
The process begins with an IVF cycle where mature eggs are extracted from a donor or patient. We then initiate fertilization protocols using the partner’s or donor’s sperm.
After ensuring proper embryonic development in our state-of-the-art lab, the embryos are cryopreserved (frozen) using vitrification until pregnancy is desired. When the time is right, the embryos are thawed and transferred into the uterus of the patient or a surrogate. This method allows the body to recover from the initial egg retrieval, ensuring a more receptive environment for implantation.
Natural FET vs. medicated FET: which protocol is right for you?
One of the first decisions we make together is choosing the protocol that best fits your clinical needs. There are two primary paths: a natural FET cycle and a medicated (artificial) FET cycle.
In a natural cycle, we track your body’s own ovulation and time the transfer to match your natural window of receptivity. In a medicated cycle, we use supplemental hormones to control the timing and ensure the uterine lining is perfectly prepared.
According to research published in Oxford Journals Human Reproduction Update, there is no significant difference in pregnancy rates between natural cycle FET and artificial (medicated) FET. However, the study notes that the heavy use of luteal phase support in the form of progesterone and hormone therapies in medicated cycles might contribute to certain complications like pre-term birth.
At PRC, we don’t believe in a one-size-fits-all approach; your protocol will be guided by your own clinical markers and your physician’s expert advice to maximize safety and success.
The FET timeline: step by step
Navigating the FET process is a team effort. Here is how the journey unfolds at Pacific Reproductive Center.
Step 1: Baseline monitoring appointment (cycle day 2-3)
Your cycle officially begins with a baseline appointment on the second or third day of your period. During this visit, we perform a transvaginal ultrasound and blood work to ensure your ovaries are “quiet” (no large cysts) and your hormone levels are at their baseline. This confirms that your body is ready to start the preparation phase.
Step 2: Uterine lining preparation (days 1-14 approximately)
Over the next two weeks, the focus shifts entirely to the endometrium (the lining of your uterus). If you are on a medicated cycle, you will begin estrogen therapy to help the lining thicken. We monitor your progress closely with ultrasounds. We are looking for a “trilaminar” (three-layered) appearance and a specific thickness that indicates readiness.
Step 3: Progesterone start and transfer timing
Once the lining is optimal, we introduce progesterone. This hormone is the “trigger” that tells your uterus to become receptive. The timing here is precise; the embryo transfer is typically scheduled for five or six days after you begin progesterone, depending on the stage at which your embryo was frozen.
Step 4: The embryo transfer procedure
The transfer day is a momentous occasion. In our comfortable procedure rooms, your physician uses a thin, flexible catheter to gently place the thawed embryo into your uterus under ultrasound guidance. It is a quick, typically painless procedure that requires no anesthesia.
Step 5: The two-week wait and pregnancy test
Perhaps the most challenging part of the process is the “two-week wait.” During this time, you continue your hormone support. Approximately 10 to 14 days after the transfer, you will return to PRC for a blood pregnancy test (beta hCG). Our team stays in close contact with you during this time, providing the emotional support you need while waiting for the results.
How long does a FET cycle take overall?
A frozen embryo transfer (FET) cycle usually takes three to six weeks from the start of your period to the day of your embryo transfer. However, the exact timing varies based on your body, the type of plan your doctor uses and whether you need extra steps like genetic testing.
At Pacific Reproductive Center, your care team will map out a timeline that fits you. They will review your medical history and monitor your progress with simple blood tests and ultrasounds. This helps them check your hormone levels and make sure your uterine lining is ready. Your medications and schedule may be adjusted along the way, so your treatment stays on track and is tailored to your body.
Tips for a successful frozen embryo transfer cycle
Beyond the clinical steps, there are meaningful ways to support your cycle:
- Prioritize rest and stress reduction. Chronic stress can affect hormone levels and immune function, so gentle practices like yoga, meditation or acupuncture may help.
- Follow your medication protocol precisely. Timing and dosing of progesterone and estrogen are carefully calibrated, so consistency matters.
- Nourish your body. A balanced, anti-inflammatory diet rich in whole foods and lean protein supports reproductive health.
- Avoid smoking, alcohol and excess caffeine. These substances can negatively impact implantation and early embryonic development.
- Attend all monitoring appointments. These visits allow your care team to catch and respond to any issues early.
Frequently asked questions about the FET timeline
How long does a frozen embryo transfer cycle take from start to finish?
Most FET cycles take two to three weeks from the start of your period to your pregnancy test. Timelines vary based on your body’s response to preparation and your individual protocol.
What is the difference between a natural and medicated FET cycle?
A natural cycle relies on your own ovulation, while a medicated cycle uses estrogen and progesterone to control the timing and lining development. Both are highly effective.
How thick does the uterine lining need to be for embryo transfer?
Most fertility specialists look for an endometrial thickness of at least 7-8mm before proceeding with transfer, as this is generally associated with optimal implantation conditions. Your Pacific Reproductive Center physician will monitor your lining closely and advise you on your specific target.
Is bed rest required after a frozen embryo transfer?
Strict bed rest is not typically recommended. Light activity is fine, though strenuous exercise should be avoided for a day or two. Your care team will provide specific post-transfer instructions.
When will I know if my FET worked?
We schedule your blood pregnancy test roughly 10–12days after the transfer. This test is much more accurate than over-the-counter urine tests.
What are the success rates for frozen embryo transfer at Pacific Reproductive Center?
PRC’s live birth success rates are consistently above the industry average, thanks to our advanced freezing techniques and individualized protocols. During your consultation, we provide specific success estimates based on your age and embryo quality.
